To obtain a copy of medical records on behalf of a patient, please submit a hand-signed HIPAA-compliant Authorization to Release Protected Health Information using one of the following methods:

Requests for patients formerly seen by Anchor Medical will be serviced by AMS starting July 1, 2025. Please label release forms with Anchor Medical's practice name and address: 180 Corliss St, Providence, RI 02906; phone: (401) 793-8400. Previously completed record releases completed on Anchor's form can also be faxed to the number above.
